I remember the Gallwey book well!

More recently, Don Greene has picked up a following, and has some  
great advice—from specific preparation lists to more general advice— 
in his masterclasses and books. (One of the topics he speaks a great  
deal on is what Maltz writes about: the self-talk inner dialogue; he  
references Maltz a few times.)
