Dan,

I agree with Dave - First make sure it isn't a rotator cuff problem - then 
establish a regimen of light weight lifting and stretching. If you have a 
really good health insurance policy I recommend seeing a good Palmer Method 
Chiropractor to get a good progressive exercise regimen started. This worked 
for me but without the insurance the cost is prohibitive. If you are 
interested in the types of exercises I used let me know off-list.
